For Lebanese:
➢ Individual Copy of Extract of Civil Registration (ikhraj kaid fardi)
➢ Birth certificate
➢ Divorce certificate (if any)

For foreigners:
➢ Individual Copy of Extract of Civil Registration, certified by the Ministry of
Foreign Affairs and Armenian Embassy in Relevant country (Apostilled)
